Suggested Documentation It is suggested that all internships and co-ops affiliated with the University of California, Riverside, are documented. Either a Learning Agreement (available on-line or supplied by the student) or an Academic-Credit Form 198I (supplied by the student) should be filled out and signed by the intern and the site supervisor and returned to the Career Center at the start of the student's internship or co-op experience. The student is responsible for providing the appropriate document.
Near the end of the student's internship or co-op experience, the site supervisor is asked to complete the Evaluation of Student Intern form, and either FAX (951.827.2447) or mail it to the Career Center.
